PETITION FOR APPOINTMENT OF A GUARDIAN
FOR MINORS OVER THE AGE OF FOURTEEN YEARS
To the Honorable P.V.MARINO,President Judge of said Court:
The Petition of William James Jurick and Ralph Everett
Jurick respectfully represents:
.T~'That:they .are the children of ANNA MAE JURICK,who
died testate on J~ly 2,1967;and that the father of said childre.
died pri9r thereto.'
2.That they.reside with their aunt,Mary Pelhan,a:t 125.,
Jonathan Drive,McMurray,Peters Township,Washington County,
Pennsylvania.
3.That your petitioners are over fourteen years of age;
the said William James Jurick having been born on July 27,1951;
and the said Ralph Everett Jurick having been born on January 2,
1953.
4.That your petitioners are the sole beneficiaries named
in the life insurance policy taken out by their mother,Anna Mae
Jurick,with the Paul Revere Life Insurance Company;said policy
being known as'life policy #96618 and being in the amount of
$1,000.00.
5.That in her Will dated April 12,1967,Anna Mae Jurick
bequeathed and devised all of her estate,real,personal and
mixed,to the aforesaid two children and appointed Mary Pelhan
as the testamentary guardian of the estates of said two children..,.

,'6.That the said Paul Revere Life Insurance Company will
pay the proceeds of the aforesaid life policy only to a guardian
appointed by your honorable court.
7.That the proposed guardian,namely;Mary Pelhan,has
no interest,whatsoever,adverse to your petitioners,except a

claim for $~Yo -advanced for ther,pay~ent of the funeral
bills of her sister,Anna Mae Jurick,the mother of the aforesaid
children.
WHEREFORE,the petitioners pray Your Honorable Court to
appoint their aunt,Mary Pelhan,as guardian,to serve without
filing any bond,and they will ever pray.

be my Last Will and Testament,in manner and farm foHowing,hereby revoking any will or wills
heretofore made by me.
First.I direct that all expenses attributable to my last illness and funeral be fully paid and
satisfied,as soon as conveniently may be,after my decease.
Second.I give,bequeath and devise all of my estate,real,per-
sonal and mixed,to my beloved sons,Ralph Jurick and William.Jurick,
equally,share and share alike.

I do hereby make,constitute and appoint Mary Pelhan,-----------------------
to be my eJQecut rix of this my Last Will and Testament,and also Testamentary
Guardian of the estates of any minor distributees during their respective
minorities;if Mary Pelhan be deceased at the time of my death,then,UNION
NATIONAL BANK OF PITTSBURGH shall be the Executor of this my Last will and
Testament;and also Testamentary Guardian of the estates of any minor
distributees during their respective minorities.
IN WITNESS WHEREOF,I,ANNA MAE JURICK,widow,the Testat rix
above named,have hereunto subscribed my name and affixed my seal,the 12th
day of April in the year of our Lord one thousand nine hundred and sixty-seven
