PETITION FOR APPOINTMENT OF GUARDIAN
FOR THE ESTATE OF JAMES NOVAK,
a minor
TO THE HONORABLE P.VINCENT MARINO,PRES[@ENTTJUDGE OF THE SAID
COURT:
1.The Petitioner is the mother of James Novak,10
years of age,he having been born on May 20,1958 and she resides
at 361 Vercheck Drive,Houston,Washington County,Pennsylvania.
2.That it is necessary to appoint a Guardian for the
estate of the said minor for the reason that he is to receive the
sum of TWO IDH~USAND FIVE HUNDRED TWO and 23/100 ($2,502.23)
DOLLARS in the settlement of a Trespass action.
3.That the said minor has no Guardian of his estate.
4.The name and address of the proposed Guardian of
the said minor's estate is Mike Zaricki,604 Moore Avenue,Canons-
burg,Washington County,Pennsylvania.
5.That the proposed Guardian has no interest adverse
to the minor and is not the fiduciary or officer or employee of
a corporate fiduciary of any estate in which the minor has an in-
terest.
6.The said minor is entitled to the sum of $2,502.23
which sum is due him as a compromise settlement in a Trespass
action in the Court of Common Pleas of Washington County,Pennsyl-
vania,at No.Term,1968,A.D.The sum afore-
said are all the assets of the minoris estate.

WHEREFORE,your Petitioner prays your Honorable Court
to appoint MIKE ZARICKI,as the Guardian of the said minor's
estate.
ACCEPTANCE OF GUARDIAN
I,the undersigned,agree to accept the appointment of
Guardian of the Estate of James Novak,a minor.

AND NOW,this ~day of
foregoing Petition presented in open Court,

Novak,appearing in open Court makes choice of Mike Zaricki,and
upon consideration thereof,Mike Zaricki is hereby appointed
Guardian of the Estate of the said James Novak as prayed for.
AND IT IS FURTHER DIRECTED that the sum of TWO THOUSAND
FIVE HUNDRED TWO and 23/100 '~$2,502.23)DOLLARS shall be paid to
Mike Zaricki as Guardian of James Novak,to be deposited by him
in the Sloven ian Savings and Loan Association of Canonsburg,Penn-
sylvania,said sum to remain in said account and not to be with-
drawn therefrom during the minority of said minor without prior
Orphans'Court approval,and the said Slovenian Savings and Loan
Association of Canonsburg,Pennsylvania,is hereby authorized to
pay over the said sum of TWO THOUSAND FIVE HUNDRED TWO and 23/100
($2,502.23)DOLLARS plus accrued interest directly to the said
minor upon proper proof that the said minor has attained his
majority.
