PETITION FOR APPOINTMENT OF GUARDIAN
FOR THE ESTATE OF GERALD LEROY
HOLLIS,JR.,a minor
TO THE HONORABLE P.VINCENT MARINO,PRESIDENT JUDGE OF THE SAID
COURT:
1.The Petitioners are the parents of Gerald Leroy
Hollis,Jr.,4~years of age,he having been born on September 26,
1963 and they reside at 245 McClane Farms Road,Washington,Wash-
ington County,Pennsylvania.
2.That it is necessary to appoint a Guardian for the
Estate of the said minor for the reason that he is to receive the
sum of FOUR THOUSAND and no/IOO ($4,000.00)DOLLARS in the settle-
ment of a Trespass action.
3.That the said minor has no Guardian of his Estate.
4.The name and address of the proposed Guardian of the
said minor's Estate is Anna Hollis,R.D.#3,stoystown,Pennsyl-
vania.
5.That thepropo~ed Guardian has no interest adverse
to the minor and is not the fiduciary of officer or employee of
a corporate fiduciary of any estate in which the minor has an in-
terest.
6.The said minor is entitled to the sum of $4,000.00
which sum is due him as a compromise settlement in a Trespass ac-

tion in the Court of Common Pleas of Washington County,Pennsyl-
vania,at No.Term,1968,A.D.The sum afore-
said are all the assets of the minor's Estate.
WHEREFORE,your Petitioners pray your Honorable Court to
appoint .ANNA HOLLIS,as the Guardian of the said minor's Estate.

Hollis,Jr.,appearing in open Court makes choice of Anna Hollis
and upon consideration thereof,Anna Hollis is hereby appointed
Guardian of the Estate of the said Gerald Leroy Hollis,Jr.as
p~ayed for.
AND IT IS FURTHER DIRECTED that the sum of FOUR THOUSAND
and no/IOO ($4,000.00)DOLLARS shall be paid to Anna Hollis as
Guardian of Gerald Leroy Hollis,Jr.,to be deposited by her in
the Slovenian Savings and Loan Association of Canonsburg,Pennsyl-
vania,said sum to remain in said account and not to be withdrawn
therefrom during the minority of said minor without prior Orphans'
Court approval,and the said Slovenian Savings~and Loan Associatio
of Canonsburg,Pennsylvania,is hereby authorized to pay over the
said sum of FOUR THOUSAND and no/IOO ($4,000.00)DOLLARS plus
accrued interest directly to the said minor upon proper proof that
the said minor has attained his majority.
