AND NOW THIS 1/~~y of April,1968,upon
consideration of the foregoing petition and upon motion of
Sherman H.Siegel,attorney for petitioner,the Court authorizes
the payment of the proceeds of Policy No.1091090 issued by The
Union Central Life Insurance Company upon the life of Clark P.
Hunter to be delivered to and received by Evelyn Hunter,natural
guardian of Ray Clark Hunter,a minor,beneficiary named in said
policy,without the appointment of a guardian or the entry of
security;and a certified copy of this Decree shall constitute
sufficient authority to the insurer to recognize the said Evelyn
Hunter as entitled to receive said proceeds and said Decree shall
in all respects have the same force and effect as an instrument
executed by a duly appointed

PETITION OF NATURAL GUARDIAN
OF MINOR TO RECEIVE PROCEEDS
OF LIFE INSURANCE
TO THE HONORABLE PRESIDENT JUDGE OF SAID COURT:
The petition of Evelyn Hunter respectfully
represents:
1.That she is the Mother of Ray Clark Hunter
who was born July 11,1954,and that said minor resides with your
petitioner at R.D.#3,Claysville,Pennsylvania (South Franklin
Township,Washington County).
2.That said minor is the beneficiary named in
Policy No.1091090 issued by The Union Central Life Insurance
Company on the life of Clark P.Hunter,Grandfather of said minor.
3.That the said Clark P.Hunter died on
February 20,1968,and that the proceeds of said policy payable
to said minor amount to $1,000.00.
4.That the insurer has re.quested that a proper
decree of Court be obtained appointing a guardian for said minor
with authority to execute a proper receipt to said insurer.
5.That said minor has no other estate or
interest in any estate other than the foregoing.
WHEREFORE,your petitioner prays Your Honorable
Court that the proceeds'of said life insurance policy be paid
and delivered to your petitioner as natural guardian of said
minor,Ray Clark Hunter,in accordance with Section 1001 of the
Fiduciaries Act of 1949 without the appointment of a guardian
or the entry of security,and that your petitioner be authorized
by Your Honorable Court to execute as natural guardian,any
receipt or other appropriate instrument necessary for the
purposes set forth above.

PETITION FOR DISTRIBUTION
The petition of Ray Clark Hunter respectfully represents:
1.That in the above captioned proceeding,Your Honorable
Court by Decree dated April 22,1968,directed the payment of life
insurance proceeds of which.your petitioner was the beneficiary
to his Mother,Evelyn Hunter,and directed that same be deposited
in a savings account with the restriction that no withdrawals
shall be made unless authorized by the Court.
2.That the said Evelyn Hunter,natural guardian of
petitioner,deposited said funds in an account in the name of Evel n
Hunter,Guardian of Ray Clark Hunter,a minor,in the First
Federal Savings and Loan Association of Washington,Pennsylvania,
being Savings Account No.24223.
3.That your p~titioner has reached the age of twenty-one
years having been born July 11,1954,an official copy of his
will be presented to the Court for inspection.

4.That there is a balance of $1445.07 in said savings
account as of June 30,1975.
WHEREFORE,petitioner prays Your Honorable Court to direct
payment of said savings Account to your petitioner.
