PETITION OF NATURAL GUARDIAN
OF MINOR TO RECEIVE PROCEEDS
OF LIFE INSURANCE
TO THE HONORABLE PRESIDENT JUDGE OF SAID COURT:
The petition of Charles W.Hamilton respectfully
represents:
1.That he is the Father of David W.Hamilton who
was born June 29,1959,and that said minor resides with your
petitioner and wife at 72 Willow Drive,Washington,Pennsylvania
(Amwell Township,Washington County).
2.That said minor is the beneficiary named in
Policy No.J1251309 issued by united Insurance Company of America
on the life of Daniel Houlihan,cousin of said minor.
3.That the said Daniel Houlihan died on May 24,
1967,and that the proceeds of said policy payable to said minor
amount to $401.50.
4.That the insurer has requested that a proper
decree of Court be obtained appointing a guardian for said minor
with authority to execute a proper receipt to said insurer.
5.That said minor has no other estate or interest
in any estate other than the foregoing.
WHEREFORE,your petitioner prays Your Honorable
Court that the proceeds of said life insurance policy be paid
and delivered to your petitioner as natural guardian of said
minor,David W.Hamilton,in accordance with Section 1001 of the
LA'
Fiduciaries Act of 1949 without the appointment of a guardian
or the entry of security,and that your petitioner be authorized
by Your Honorable Court to execute as natural guardian,any
receipt or other appropriate instrument necessary for the
purposes set forth above.

AND NOW THIS 1:~ay of April,1968,upon
consideration of the foregoing petition and upon motion of
Sherman H.Siegel,attorney for petitioner,the Court authorizes
the payment of the proceeds of Policy No.J 1251309 issued by
united Insurance Company of America upon the life of Daniel
Houlihan to be delivered to and received by Charles W.Hamilton,
natural guardian of David W.Hamilton,a minor,beneficiary named
in said policy,without the appointment of a guardian or the
entry of security;and a certified copy of this Decree shall
constitute sufficient authority to the insurer to recognize the
said Charles W.Hamilton as entitled to receive said proceeds
and said Decree shall in all respects have the same force and
effect as an instrument executed by a duly appointed guardian
under Court Decree.

PETITION FOR PAYMENT OF FUNDS
TO THE HONORABLE JUDGES OF SAID COURT:
The petition of David Hamilton,respectfully
represents:
1.Petitioner resides at 72 Willow Drive,Washington,
Pennsylvania.
2.By Decree of Your Honorable Court dated November
20,1968,in the Estate of D.J.Houlihan filed at No.1005 of
1968,the Court awarded to Petitioner,who was then a minor,
his distributive share in said estate amounting to $382.08
with the restriction that same be deposited in First Federal
Savings and Loan Association of Washington,Pa.,said funds
not to be withdrawn,neither principal nor interest,until said
minor reached the age of twenty-one years.
3.Your petitioner became twenty-one years of age on
.June 29,1980.
4.As of March 31,1979,the funds held in Petitioner's
Account #25196 at First Federal Savings &Loan Association of
Washington,Pa.amounts to $1732.74.
WHEREFORE,Petitioner requests Your Honorable Court to
