condition, is unable to manage his property or is liable to dissipate it and
become the victim of designing persons.

AND NOW., comes the PITTSBURGH NATIONAL BANK.,
Guardian of the Estate of Charles Reed Reynolds., also known as C. R.
Reynolds., an incompetent, and respectfully petitions your Honorable Court
as follows:
1. That your Petiti'oner was duly appointed as Guardian of
the Estate of Charles Reed Reynolds, a/k/ a C. R. Reynolds, an incompetent.,
by Order and Decree of your Honorable Court on February 14., 1968.
2. That after the appointment of your Petitioner as Guardian
of the Estate of Charles Reed Reynolds, a/k/a C. R. Reynolds, an incompetent,
a considerable amount of time and effort had to be expended in order to obtain
the assets of the incompetent., which assets were receipted for by the said
Guardian on or about October 7., 1968.
3. . That your Petitioner has in its possession assets belonging
to the said incompetent in the approximate sum of Fifty Thousand ($50., 000. 00)
Dollars ..
4. That your Petitioner has been requested to pay certain
bills incurred on behalf of the Estate of Charles Reed Reynolds, a/k/a C .. R.
Reynolds., an incompetent, from the cash assets of the said estate ..

maintenance of the said incompetent in her home., and
who provides for the incompetent aU food, clothing.,

is necessary, for the period from February 1968 to

represents a charge of$ 150.00 per month ...
and a request that a sum of$ 150.00 per month
be paid to her in the future for the care of the said
incompetent.

6. That it is the opinion of your Petitioner .. Guardian of the Estate

charges requested are fair and reasonable for the services that have been
rendered., and it is further the opinion of the Guardian that a future monthly
allowance of$ 150.00 per month is fair and reasonable for the proper
care and maintenance of the said incompetent.
WHEREFORE, Petitioner respectfully prays your Honorable
Court to make an Order permitting the said Pittsburgh National Bank, Guardia
of the Estate of Charles Reed Reynolds;.o a/k/a C. R .. Reynolds, an incompetent.,
to pay from the cash assets of the said estate the sums requested above.
